Qualifications:
- Graduate of an accredited school of nursing.
- Holds a current valid license in good standing as a Registered Nurse by the Board of Nursing.
- A minimum of one year experience with six months supervising paraprofessional caregivers.
- Excellent communication (verbal and written) interpersonal, organizational, and leadership skills.
- Ability to communicate effectively with a wide variety of individuals, including clients and their families, physicians, office staff and other health providers/partners.
- Experience in Geriatric Nursing is a plus
- Must have a valid driver's license and transportation and agree to a full background check.
- Must provide a completed application with past or current work history, and professional/personal references must be submitted.
Work Hours:
- This is a Part-Time position requiring 10-15 hours per week.
- Occasional evenings, weekends, or holidays may be needed.
- On call duties will be routinely required.
Responsibilities Include:
- Review client records for completeness and compliance to State regulations and Company policy.
- Responsible for the review of Caregiver charting to determine its accuracy and consistency with the individual plan of care for each client.
- Regular supervisory visits with clients and caregivers.
- Conduct periodic skills testing with caregivers. Determines competency and skills of caregivers for the purpose of matching client need to staff ability and meeting all the needs of the client.
- Perform initial assessments for clients to capture critical information and develop the Individual Service Plan as required.
- Collaborates with other disciplines in discharge planning as required.
- Participates in data collection for Performance Improvement activities.
- Evaluates the quality of care on an ongoing basis and monitors for compliance with current standards of care.
Compensation and Benefits:
- $35-$40 per hour, based on experience + commission
- PTO days accrued with hours and tenure
