Key Responsibilities:
- Client Record Review: Ensure client records are complete and compliant with state regulations and company policies.
- Caregiver Supervision: Review caregiver charting for accuracy and consistency with the client's individualized care plan.
- Supervisory Visits: Conduct regular visits with clients and caregivers to ensure high-quality care and satisfaction.
- Skills Testing: Perform periodic skills testing to assess caregivers’ competency and match client needs with caregiver abilities.
- Initial Assessments: Conduct assessments for new clients, gather critical information, and develop an Individual Service Plan (ISP) as required.
- Collaboration: Work with interdisciplinary teams to contribute to discharge planning and coordinate care with other healthcare providers.
- Performance Improvement: Participate in data collection for performance improvement initiatives.
- Ongoing Care Evaluation: Monitor and evaluate the quality of care on an ongoing basis to ensure compliance with established standards.
- Compliance and Monitoring: Ensure that caregivers meet client care needs effectively and adhere to standards of care.
Qualifications:
- Education: Graduate of an accredited nursing program.
- Licensure: Current, valid Registered Nurse license in good standing with the Board of Nursing.
- Experience: Minimum of one year of nursing experience, with at least six months of experience supervising paraprofessional caregivers.
- Communication Skills: Strong verbal and written communication skills with the ability to communicate effectively with clients, families, physicians, and other healthcare professionals.
- Organizational & Leadership Skills: Excellent organizational skills and the ability to provide effective leadership to caregivers.
- Transportation: Valid driver's license and reliable transportation required.
- Background Check: Must agree to a full background check.
Work Hours:
- This is a part-time position requiring 10-15 hours per week.
- On-call duties will be routinely required.
Compensation and Benefits:
- Pay Range: $45 - $50 per hour, based on experience.
